SHAPE (System for Health Assessment and Population Data Evaluation)

The System for Health Assessment and Population Data Evaluation (SHAPE) is a data integration system designed to monitor key metrics for clinical and community-based populations for the expanded HCI Area We Serve.

SHAPE is developed by the HCI Population Sciences Research Infrastructure, Systems & Management (PRISM) with support from CODE.

  • Browse the Cancer InFocus app to access publicly available data for the Area HCI Serves.

  • If you have a visual request, or would like different data, or have general questions, please submit a request via SHAPE data request.

  • Visit SHAPE on PULSE for more information on SHAPE.
